Job Description:

A company is looking for a Care Manager (RN) OB.
Key Responsibilities

Develops and facilitates complex care management activities and personalized care plans for members
Coordinates care between members, families, and healthcare providers to ensure effective service delivery
Monitors member progress and revises care plans as necessary to meet goals and address unmet needs

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure and/or Compact State Licensure required
Maternal Health or OB experience preferred
