Job Description:
A company is looking for a Behavioral Health Case Manager.
Key Responsibilities
Monitor and manage reports to identify gaps in care for Follow-Up
After Hospitalization (FUH)
Contact members to complete Behavioral Health HEDIS assessments
Document assessments and assist members with scheduling follow-up
appointments
Required Qualifications
Bachelor's degree or equivalent experience with clinical
certification
Clinician license required: LPC, LSCSW, LMSW, or RN with Behavioral
Health clinical certification
0-2 years of experience in behavioral health (e.g., crisis
intervention, case management, therapy, intake/assessment)
Experience with medical records/EHR systems
Strong proficiency in Excel, Outlook, and other computer
systems
