Licensed Social Worker Case Manager

Job Description:

A company is looking for a dedicated Social Worker Case Manager to join the team.
Key Responsibilities

Assess patients' biopsychosocial needs and communicate with patients, families, and care teams regarding care plans
Ensure effective communication of care options and advocate for patients to resolve barriers to care progression
Provide discharge planning and coordinate post-discharge services in collaboration with interdisciplinary care teams

Required Qualifications

Part-time weekend work schedule (Saturday and Sunday, 8:00 am - 6:30 pm)
Masters Degree in Social Work
One year of experience in clinical care or clinical case management
Basic Life Support certification through the American Heart Association
