Builds relationships with other clinicians involved in the patients care.  Promotes patient-centered approach to care.  Partners with healthcare team to manage transitions of care between hospital, primary and specialty care.  Facilitates proactive discharge planning or follow-up to promote better outcomes, decreased length of stay and reduced readmission rates. Schedule:  This is a Full-time Nights position where you will work 12-hr shifts Tuesday - Sunday every other week. Six Nights shifts required while the seventh night (Monday) is an optional overt time shift available for call but is not mandatory. Overall responsibilities include but are not limited to:  Provides appropriate patient care for patients with hematologic and oncologic malignancies in the ICU or step-down unit to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation.  Educate patients in areas of procedures, pharmacology, self-management of side effects and available community resources.  Provides symptom management for treatment related toxicities.
